Jay is the founder of Re.Create Longevity Training, a unique blend of one-on-one, concierge-level care and cutting-edge digital services designed to prevent injury, improve performance, and support long-term health. His approach combines evidence-based rehabilitation with advanced performance tools - including body composition analysis, force plate testing, and individualized movement programs - giving clients the insight and strategies they need to train smarter and live stronger.
Before launching Re.Create, Jay most recently helped build and lead 20 physical therapy clinics across 3 time zones, 11 of those clinics specific to the Portland metro area. Jay developed a reputation for delivering high-quality care and creating lasting client relationships. Today, he works with athletes, professionals, and everyday movers who want more than quick fixes - they want lasting results.

Adam was born and raised in Seattle, WA. He attended Fordham University in NYC for his Bachelor’s in Biology, and received his Doctorate in Physical Therapy (DPT) from USC.
Growing up, Adam was a multi-sport athlete, playing Football and Lacrosse for his high school. He continued his passion for sports and started his journey towards rehabilitative medicine working as a student athletic trainer with the Fordham football team, providing care in the training room and on the field. Through his journey he developed a passion for helping people reach their peak levels of performance and live their lives to the fullest.
Adam has a special interest in sports performance training in addition to physical therapy. He has worked with athletes of all levels from recreational and youth athletes, weekend warriors, all the way up to the pros in the NFL, NBA, and MLB. He is a Certified Strength & Conditioning Specialist (CSCS), Titelist performance institute certified, and has done extensive training on movement and running analysis and rehab.
Adam works hand-in-hand with each and every patient to achieve their rehabilitation, sport performance and fitness goals. His deep knowledge and positive energy empower them through recovery, and back to the activities they love and beyond.
Outside of work you can find him playing basketball, hiking, working out, on the golf course, or outside manning the grill!.
